Membership Application - NOTE: AGM voting privilege requires membership in Hospice King-Aurora-Richmond Hill of which the membership requirement is a minimum of 10 hours of volunteer service or a minimum donation of $100.00 from April 1, 2016-March 31, 2017. Members must be 18 or older. You can submit your Membership Application via mail (350 Industrial Pkwy South, Aurora, ON L4G 3V7), drop off at office, fax: 905-727-1134, scan & email events@hospicekingaurora.ca or complete the form on the day of the meeting. Membership is for one fiscal year (April 2, 2017-March 31, 2018).
